If you are already registered for the upcoming conference, please take a moment to complete the following form so that we may create a face book of attendees for later networking and resource sharing. NOTE: Your entry must be submitted by Feb. 17 or it will not appear in the face book. No exceptions.

Submitted information will be held in confidence and the face book will only be distributed at the conference to you and your fellow attendees. Please note that two separate steps are required to complete a submission for the face book.

STEP ONE: Please send an email with your picture (or your organization logo) attached as a .jpg or .gif file to killina@openhearth.org. In your email, please also indicate your name (and craft name, if you use one) and primary group affiliation so we can more easily match your picture to the information submitted on this form.

STEP TWO: Please complete the following form. Please limit your bio descriptions to a very BRIEF 300 words or less. Longer submission will be edited for size, and we won't always know what part of a longer submission is most important to you: Brevity is encouraged.
